Recovering from Post-Viral Fatigue, Burnout & Chronic Low Energy | Nutritionist & Naturopath Surrey
In this blog, Natalie from @natnuthc Surrey’s leading Naturopathic Nutritionist and Functional Medicine test export deep dives into post-viral fatigue, burnout and low energy.
If you’ve found your way here, chances are your energy hasn’t come back the way you expected it to.
For some people it starts with a virus such as COVID, glandular fever, a respiratory infection, or “a bug” that never quite cleared. For others it follows a prolonged period of stress such as divorce, bereavement, ongoing pressure at work, financial strain, caregiving, or emotional overload, and very often it can also be a combination of both; the result is a system already under strain that then gets tipped over the edge by an infection that it can’t fully recover from.
There’s also a very common pattern seen in clinic where this shows up around perimenopausal age.
Hormonal fluctuation during perimenopause doesn’t cause fatigue on its own, but it can significantly reduce resilience. This applies most commonly to women, but not exclusively. In both men and women, the underlying issue is often the same: the stress system and hormonal system become misaligned, leaving the body struggling to maintain energy, sleep, mental resilience and recovery, and the consistency of the symptoms remain including:
- Low energy that doesn’t improve with rest
- Brain fog and reduced mental clarity
- Sleep that feels unrefreshing
- Exercise intolerance or post-exertional crashes
- Mood changes or emotional flatness and outbursts
- Gradual weight gain despite no obvious lifestyle change
- Fertility issues
Most people are told their bloods are “fine” and on paper, they often are. But functionally, the body is telling a very different story. This is not just tiredness; it’s a multi-system breakdown in how the body produces, regulates, and recovers energy.
The reason, you are told your bloods are fine; is because conventional medicine is usually only interested in a diagnosis and thus if your bloods don’t fall within a certain range then you are free of a diagnosis and thus technically your are ‘fine’! However, that doesn’t mean you don’t need support and it also doesn’t mean you are not heading towards a future diagnosis. This is why naturopathic and functional medicine differs; we look at where you are heading, what is showing up that is slightly off that could lead to a bigger problem later on, and regardless of what your bloods tell us, if you are presenting with inflammation and mitochondrial dysfunction from a clinical perspective then that doesn’t mean those things are not going on, it just means it’s not yet showing up on your bloods; and we never ignore that.
What causes post-viral fatigue, chronic fatigue & burnout?
Whether we label it post-viral fatigue, ME/CFS-type patterns, or burnout with physiological collapse, the same core systems tend to be involved.
At the centre of it, several interconnected systems become dysregulated including:
- HPA axis dysfunction (stress response system imbalance)
- Mitochondrial downregulation (reduced cellular energy production)
- Immune activation (low-grade, ongoing inflammation)
- Gut disruption (microbiome imbalances driving immune signalling)
- Gut lining compromise (aka leaky Gut driving inflammation and poor recovery)
- Blood sugar instability (inconsistent energy availability and food cravings)
- Neuroinflammation (affecting cognition, mood, and clarity)
It is rarely one isolated issue, instead it is a network of systems stuck in a protective, energy-conserving state. We quite often refer to this as ‘survival mode’; the biology is telling you to stop and put its foot on the brake, but you live a busy life, you have bills to pay and a family to look after for example so you can’t just stop. Instead, we need to find a way of helping you apply that brake so you can fully recover, while still being able to carry on and meet your daily obligations, that’s the art of how we support you.
Stress, the HPA axis & why you feel wired, tired and flat
The HPA axis (hypothalamus–pituitary–adrenal system) is your central stress response system. It regulates cortisol rhythm, energy availability, and how the body responds to both physical and emotional demands, and it helps control everything from thryoid function to hormone regulation and stress management.
The important point about the HPA axis is that the system does not distinguish between types of stress. A virus, emotional trauma, grief, long-term pressure, burnout, or hormonal transition all register in the same biological language, so when the system becomes dysregulated, people often describe:
- Exhausted but unable to sleep properly
- Waking unrefreshed
- Afternoon energy crashes
- Needing caffeine just to function
- Anxiety, emotional flatness and mood changes
- Hormonal imbalance
- Fertility struggles
This is one of the most common presentations seen in post-viral fatigue and burnout cases in clinical practice in Surrey and across the UK.
Hormones, perimenopause & fatigue (energy, stress & resilience link)
One of the most overlooked layers in chronic fatigue is also hormonal transition. Specifically, around perimenopausal age, fluctuating oestrogen and progesterone levels can reduce ones resilience to stress. This doesn’t mean hormones are the root cause of the issue, but they amplify an already strained system, and when combined with HPA axis dysregulation, this can lead to:
- More pronounced fatigue and energy crashes
- Disrupted or light sleep
- Increased anxiety or low mood
- Angry outbursts and poor stress tolerance
- Reduced exercise tolerance
- Changes in body composition and weight regulation
Men can experience similar patterns too, particularly when testosterone levels decline alongside chronic stress. Some call this the Andropause!
Mitochondrial dysfunction: why energy production stops working properly
At a cellular level, energy is produced by the mitochondria. When these systems are under strain from infection, inflammation, stress, or nutrient depletion, the energy output mechanism becomes reduced, and this often presents as:
- Exercise intolerance or post-exertional crashes
- Heavy limbs and muscle fatigue
- Slow recovery from activity
- Brain fog and low mental stamina
- Feeling completely depleted after minor exertion
- Walking into a room and wondering why you are there!
This is not deconditioning, it is reduced mitochondrial cellular energy capacity.
Gut health, inflammation & immune dysregulation
The gut plays a central role in immune regulation, inflammation, and energy production as well.
When gut function is compromised, it can drive systemic issues even without obvious digestive symptoms. Some of the most common features include:
- Bloating or food sensitivities
- Fatigue after meals
- Histamine-type reactions e.g. itchiness, puffiness, watery eyes for example during or after eating.
- Poor recovery from illness
- Ongoing low-grade inflammation causing achy joints and muscles
This keeps the immune system in a semi-activated state, which directly impairs energy recovery, disrupts hormone balance and keeps you in a vicious cycle of poor health.
Blood sugar imbalance, weight gain & fatigue
A very common and frustrating pattern we also see with persistent fatigue is gradual weight gain and mood changes.
This is often driven by blood sugar instability. When blood sugar is not well regulated, the body compensates through stress hormones such as cortisol, which temporarily stabilise energy but worsen long-term regulation. Over time this can contribute to:
- Energy crashes throughout the day
- Poor sleep quality or early waking
- Mood fluctuations or low motivation
- Increased fat storage, particularly around the abdomen
This is not simply a dietary issue, instead it is a stress-metabolic mismatch.
Neuroinflammation: brain fog, mood & cognitive fatigue
When inflammation is present in the body, it also affects the brain, and often presents as:
- Brain fog or reduced clarity
- Low motivation or drive
- Anxiety without clear cause
- Emotional sensitivity or overwhelm
- Difficulty sustaining focus
The brain prioritises survival over performance when the system is under stress meaning memory, mood issues, focus and motivation suffer.
Why recovery doesn’t happen on its own
Over time, all of the interconnecting systems explained above that are impacting the Gut, hormones, inflammation, brain, immune function and adrenals become self-perpetuating. You try to do your research and help yourself, but you get lost in where to start. You don’t know whether to support your Gut, your hormones, your immune function, nor what to start with first, and often you end up trying to address all of it at once and this in itself becomes overwhelming both physically and mentally and rather than helping, it worsens the situation. As such, and despite trying to help yourself, the body remains stuck in a loop of stress activation, immune signalling, disrupted sleep, and reduced energy production.
Clinical approach: full health assessment first
In my clinical practice at https://www.natnuthc.co.uk, the starting point is always a full 90 minute health assessment. This can be done online or in person in Surrey.
This is not a formality, in fact it is the most important part of the process because it allows me to understand the pattern behind what is going on, not just the symptoms in isolation.
It also helps me assess whether we are dealing with more of a gut issue, a stress issue, an inflammatory issue, an adrenal picture or something else and it helps me prioritise what we work with first. Now that doesn’t mean I will ignore the other things, but what it means is that I can get to the root of the problem much quicker, and if I realise that the Gut for example is the root then by focusing my attention here, I often find that the inflammation, energy, hormones and mood for example all gradually start to align and fall into place even though we didn’t specifically target them. And that’s the difference between working with a fully qualified naturopathic nutrition practitioner rather than just googling the issue or asking your mate whose into health. As a fully qualified naturopathic nutritionst I am trained and experienced in looking for clinical signs of imbalance and understanding how to prioritise them; rather than just randomly throwing everything at the problem. We can cut through the mis-information and overwhelm and help you focus on what is really going to work!
Specifically, fatigue is never a single-issue presentation. The same symptoms in two people can come from completely different underlying drivers once you look at:
- How and when symptoms developed
- The original trigger (virus, stress, grief, hormonal shift, or combination)
- Sleep quality and recovery patterns
- Gut and digestive function
- Daily energy fluctuations and crash patterns
- Stress load and resilience over time
- Age and sex of the individual and their personal circumstances, commitments and lifestyle
From this, a clinical hypothesis is formed about what is most likely driving the system imbalance, and based on this we can design a personalised approach to address ones specific issues.
Why testing is secondary to clinical reasoning
Only once the full clinical picture is clear do I consider functional testing.
Testing is not used to “find something wrong”, instead it is used to confirm, clarify, or deepen understanding of what is already clinically suspected.
If appropriate I may recommend:
- DUTCH testing to understand cortisol rhythm and hormone metabolism
- Stool testing to deepen the understanding of your microbiome balance, gut lining stability, inflammation and gut immune activity
Used this way, testing adds clinical value and helps us fine tune and specifically target our approach, but it never replaces professional and experienced clinical reasoning.

Treatment approach: restoring energy systems step by step
Once the full picture is established through assessment and targeted testing (if necessary), the focus shifts to rebuilding core systems.
This may include supporting:
- Stress response regulation (HPA axis balance)
- Gut integrity and immune function
- Blood sugar stability and metabolic rhythm
- Mitochondrial energy production and recovery capacity
The order and emphasis always depend on the individual case, and there is no fixed protocol for fatigue because no two presentations are the same.
Key takeaway
If you are dealing with ongoing fatigue after a virus, burnout, emotional stress, hormonal transition, or a combination of these factors, what you are experiencing is not simply tiredness. It is a multi-system downregulation involving the stress response system, immune function, gut health, hormones, thyroid and mitochondrial cellular energy production.
And importantly, it is not permanent, but it does require a structured, systems-based approach that starts with understanding your individual pattern and underlying root causes, and it needs an approach that is specifically designed for you based on your personal biochemistry and your personal capacity and lifestyle.
